New Utah County Health offices open in American Fork

Birth and death certificates, immunizations, and environmental health and food service permits are now available at the Utah County Health Department's new American Fork office.

"With all the growth in the northern parts of Utah County, we needed full time officers closer to the homes and businesses of citizens in those cities," said Dr. Joseph Miner, Utah County Health (UCHD) executive director.  "Birth and death certificates and immunizations are now available every business day full time in our American Fork offices."

The new office is located on the north east corner of 600 South and 500 East, just east of I-15 exit 276, in American Fork.  "We are in the same building as our American Fork WIC Clinic," said Dr. Miner.  "The location is very convenient off of the freeway."

UCHD walk-in services available to everyone in the general public include:

  • Birth and death certificates
  • Immunizations for children, adults, and travel
  • Daycare facility application/inspection requests
  • Food establishment permits (annual and temporary)
  • Food handler renewal/replacement
  • Mass gathering permits
  • Radon / meth test kits
  • Tuberculosis testing
  • Water Sample pick-up 

Appointments will be necessary for some additional services, including:

  • Child car seat and booster seat safety checks (call 801-851-7035)
  • "Stepping-On" adult fall prevention classes (801-851-7035)
  • Youth tobacco cessation classes (801-851-7095)
  • WIC Nutrition Services (801-851-7320)

"Health Promotion classes will be held at the American Fork offices in the evenings," said Miner." It will be wonderful to be able to offer such activities as healthy lifestyle classes, car seat installation classes, and our Living Well with Chronic Disease class."

The Utah County Health Department is committed to promoting the health the community and preventing avoidable disease and injury by monitoring the health of the community and assuring conditions in which people can be healthy.
